The FHPT Affiliate Appreciation Program is our way of giving back to the patients who trust us and help grow our community. By referring friends and family and leaving honest reviews, you can earn exciting rewards while supporting our mission. The program features three levels—Advocate, Ambassador, and Champion—each offering increasing benefits like in-clinic credits, exclusive apparel, discounted sessions, and recognition for your contributions. It’s our way of showing gratitude to those who help us do what we do best: deliver exceptional care.
Advocate
For those just starting to share the FHPT experience.

Continue earning $25 per referral and $10 per review until you reach Champion level.
As an Advocate, you’re taking the first steps in sharing the FHPT experience. For every friend, family member, or colleague you refer to FHPT, you’ll receive $25 in in-clinic credit, and for each honest review you leave online, you’ll earn $10 in credit. You can continue earning referral credits until you advance to the next level, making it a simple and rewarding way to share the care you love.
Ambassador
Achieve this level by referring 3 people and leaving 3 honest reviews within a year.

Reaching the Ambassador level means you’ve referred three people and left three honest reviews within a year, demonstrating your commitment to growing the FHPT community. You’ll also continue earning $25 per referral as you work toward becoming a Champion.

Becoming a Champion is an exceptional achievement, signifying your dedication and advocacy for FHPT. To qualify, you need to refer six or more people and leave five or more reviews in a year. Champions enjoy 40% off every session for the entire year, a custom Champion Shirt only available to those at this level, and a custom plaque displayed in our clinic to honor your contributions, which you’ll receive at the end of the year. You’ll also receive a social media spotlight as a public thank-you and an FHPT item of your choice from our online store (up to $200 value). Champions truly embody the spirit of our FHPT community.
